To open an Online Trading Account with DBSV, clients are required to complete the Account Registration Form below and submit it electronically to our Online Trading and Marketing Promotion Department. Upon receiving your online application, our Online Trading representative will contact you shortly to discuss the details and an account application package will be forwarded to your mailing address stated in the registration form by registered mail. |

In order to process your application, we will require the following personal documents. |

For Thai Individual clients
- A certified true copy of your Identification Card.
- A certified true copy of House Registration Certificate.
- A certified true copy of your recent bank/financial statement for the past 6 months.
- Duty stamps Baht 30.

For Foreign Individual clients
- A certified true copy of your passport.
- A certified true copy of your Work Permit, if you are currently working in Thailand.
- A certified true copy or your recent bank/financial statement for the past 6 months.
- A certified copy of your recent bank/financial statement for the past 6 months.
- Duty stamps Baht 30.

- Please be advised that applications cannot be processed without appropriate signatures. To avoid delays, please make sure every page is signed and necessary documents are complete.
- For clients residing overseas, all supporting documents should be verified by a Notary Public and Royal Thai Embassy or Thai Consular in that jurisdiction.

Upon receipt of complete account documentation, pending approval, your new Online Trading Account will be ready within about 3 days. A welcome mail containing your account number and User ID will be sent to the address provided shortly. |

For security reasons, your Password/PIN will be sent separately by registered mail to the address provided in your application. |

Please note that your are advised to change your Password/PIN on your first login to www.dbsvitrade.com. Kindly change it according to the Password Mangement Guideline. |
